You can use these steps to restrict the form responses to one per person.

On the form edit page go to Settings on the top-left corner -> General and check the option Disable Multiple Responses.

This method uses cookies to work which isn't foolproof. The user can counter this by switching to a different browser on their device, or going in private browsing mode.
