If the insured and the bank account holder is not the same, please provide ID copy of the insured, ID copy of the bank account holder and authorization letter from the insured (permitting Ulink to make the refund payment to the third party) to process the claim. There will be a link to download the authorization letter on the next page.
If the insured is under age, please provide the parent’s ID.
If the insured has submitted ID before, it is not required to submit the ID again.
ID requirement is mandated by European Law and it is required to process the claim
A separate claims form should be used for each patient and each medical condition.
Processing of this claim may be delayed if the information provided is incomplete.

အာမခံထားသူနှင့် ဘဏ်စာရင်းကိုင်ဆောင်သူသည် တူညီခြင်းမရှိပါက Claim အားဆက်လက်လုပ်ဆောင်နိုင်ရန် အာမခံထားသူ၏ ID မိတ္တူ၊ ဘဏ်အကောင့်ကိုင်ဆောင်သူ၏ ID မိတ္တူနှင့် အာမခံထားသူထံမှ ခွင့်ပြုချက်ပေးစာတစ်စောင်(ပြန်အမ်းငွေပေးချေမှုကိုဖော်ပြထားသောဘဏ်အကောင့်သို့ လွဲပြောင်းပေးပါရန် အာမခံထားသူထံမှ Ulink အား ခွင့်ပြုပေးပါ‌ကြောင်း)၊ နောက်စာမျက်နှာတွင် ၎င်းခွင့်ပြုချက်ပေးစာအား ဒေါင်းလုဒ်လုပ်ရန်လင့်ခ်တစ်ခုရှိပါမည်။
အာမခံထားသူသည် အသက်မပြည့်ပါက၊ မိဘ၏ ID ကို ပေးပါ။
အာမခံထားသူသည် ယခင်က ID တင်ပြခဲ့လျှင် ၎င်းID ကို ထပ်မံတင်ပြရန် မလိုအပ်ပါ။
ID လိုအပ်ချက်ကို ဥရောပဥပဒေက ပြဌာန်းထားသော‌ကြောင့် Claim တင်ရာတွင် မိမိတို့၏ ID နှင့်တင်ပြရန်လိုအပ်ပါသည်။
လူနာတစ်ဦးစီနှင့် ကျန်းမာရေးအခြေအနေတစ်ခုစီအတွက် သီးခြားတောင်းဆိုမှုပုံစံကို အသုံးပြုသင့်သည်။
ပေးထားသော အချက်အလက် မပြည့်စုံပါက ဤအရေးဆိုမှုကို လုပ်ဆောင်ရာတွင် နှောင့်နှေးနိုင်ပါသည်။
