Virtual Healthcare Service Agreement

Schedule [C] to Residential Lease
VIDA UNIT UPGRADES
Parties: VIDA Communities Inc. ("VIDA") and the Tenant under the Lease to which this agreement is attached as a Schedule (individually or collectively, the "Tenant”).
Background: VIDA is the agent responsible for the residential apartment property being leased by the Tenant
The Tenant has applied and been approved by VIDA to subscribe for services through the TELUS Health Virtual Care App on terms exclusive to VIDA residents.
Subscription Agreement Terms and Conditions:
1. VIDA agrees to grant the Tenant and the Tenant’s children (if any) that are identified in the Lease with access to 24/7 virtual healthcare through VIDA’s exclusive partnership with TELUS Health Virtual Care Inc. (“TELUS Health”). TELUS Health facilitates telehealth consultations, wellness resources, and other health-related services that are delivered virtually by health professionals.
2. VIDA has negotiated exclusive arrangements with TELUS Health pursuant to which VIDA residents can subscribe to TELUS Health services at a significant discount. The subscription fee charged by VIDA is C$12.99 per month (the “Subscription Fee”).
3. By entering into this agreement, the Tenant agrees to pay the Subscription Fee on the first day of each month for so long as: (i) the Lease remains in force; (ii) the Tenant is in good standing and in compliance with VIDA’s house rules; (iii) the reseller agreement between VIDA and TELUS Health remains in force; and (iv) the Tenant wishes to be a subscriber to the TELUS Health services.
If VIDA so decides (in its sole and unfettered discretion), late payment or non-payment will result in the immediate termination of this agreement and termination of access to the TELUS Health services on VIDA’s preferential terms. 
4. The Subscription Fee shall be added to the Tenant’s monthly rent and other charges payable under the Lease.
5. Except as otherwise specifically amended by the terms of this agreement, all terms, conditions and provisions of the Lease shall remain in full force and effect. In the event of any conflict between the provisions of this agreement and the Lease, the provisions of this agreement shall govern.
6. Notwithstanding anything to the contrary contained in this agreement, in no event will VIDA, its affiliates, directors, employees, contractors or agents be responsible to the Tenant or any members of its household for any indirect, special, incidental, consequential, exemplary or punitive damages or any damages for loss of profits, for loss of revenue, for loss of business opportunity or for failure to realize expected savings, whether for breach or repudiation of contract, or whether in tort, by way of negligence, gross negligence, strict liability, or otherwise, arising out of, relating to, or in connection with, this agreement or any services provided by TELUS Health, even if such damages could have been foreseen or prevented or if VIDA was advised of or knew the possibility of same. The exclusion of liability shall apply notwithstanding any fundamental breach of this agreement or any finding of unconscionability. This section shall survive the termination of this agreement.

Microwave Installation Service Agreement

Schedule [C] to Residential Lease
VIDA UNIT UPGRADES
Parties: VIDA COMMUNITIES INC. ("VIDA") and the Tenant under the Lease to which this agreement is attached as a Schedule (collectively, the "Parties”).
Background:
VIDA is the agent responsible for the residential property of this tenant.
The Tenant is currently leasing the Property pursuant to a signed lease agreement.
The Tenant has applied and been approved by VIDA to upgrade the existing property with a countertop microwave in accordance with the terms and conditions contained herein.
Agreement Terms and Conditions:
1. Installation of Microwave: VIDA agrees to install a countertop microwave in the unit of the tenant. The specific brand and model of the microwave shall be determined by VIDA in its sole discretion.
2. Service Fee: The Tenant agrees to pay an additional monthly fee of $15.99 for the microwave (the “Microwave Service Fee”) which fee shall be added to the Tenant’s monthly rent payable under the Lease.
3. Payment: The Microwave Service Fee shall be due and payable at the same time as the Tenant's monthly rent. Failure to pay the Microwave Service Fee in full each month shall be considered a default under the Lease.
4. Maintenance: VIDA is responsible for the maintenance and repair of the microwave. The Tenant agrees to notify VIDA promptly in the event of a microwave malfunction. The Tenant agrees not to tamper with, alter, or attempt to repair the microwave without the prior written consent of VIDA. The tenant agrees to clean the microwave regularly after use to keep it neat and prevent grime from forming in the microwave.
5. Lease Terms Unaffected: Except as specifically amended by this addendum to the Lease, all terms, conditions and provisions of the Lease shall remain in full force and effect. In the event of any conflict between the provisions of this addendum and the Lease, the provisions of this addendum shall govern.

Smart Lock Installation Service Agreement

Schedule [C] to Residential Lease
VIDA UNIT UPGRADES
Parties: VIDA COMMUNITIES INC. ("VIDA") and the Tenant under the Lease to which this agreement is attached as a Schedule (collectively, the "Parties”).
Background:
VIDA is the agent responsible for the residential property of this tenant.
The Tenant is currently leasing the Property pursuant to a signed lease agreement.
The Tenant has applied and been approved by VIDA to upgrade the existing entranceway locks at the Property to smart locks in accordance with the terms and conditions contained herein.

Agreement Terms and Conditions:
1. Installation of Smart Locks: VIDA agrees to install a smart lock on the primary entranceway door of the unit. The specific brand and model of the smart lock shall be determined by VIDA in its sole discretion.
2. Service Fee: The Tenant agrees to pay an additional monthly fee of $18.99 for the smart lock (the "Smart Lock Service Fee"), which fee shall be added to the Tenant’s monthly rent payable under the Lease.
3. Payment: The Smart Lock Service Fee shall be due and payable at the same time as the Tenant's monthly rent. Failure to pay the Smart Lock Service Fee in full each month shall be considered a default under the Lease.
4. Maintenance: VIDA is responsible for the maintenance and repair of the smart lock. The Tenant agrees to notify VIDA promptly in the event of a smart lock malfunction. The Tenant agrees not to tamper with, alter, or attempt to repair the smart lock without the prior written consent of VIDA.
5. Irrevocable Decision to Upgrade: The Tenant agrees and acknowledges that the installation of the smart lock is permanent and that the Tenant will be responsible for payment of the Smart Lock Service Fee for the duration of the Lease. VIDA is under no obligation to remove the smart lock and makes no guarantee as to smart lock quality or customer satisfaction.
6. Security: The Tenant acknowledges that while the smart lock is designed to enhance the security of the Property, VIDA makes no guarantees or warranties about the effectiveness of the smart lock in preventing unauthorized access to the Property.
7. Lease Terms Unaffected: Except as specifically amended by this addendum to the Lease, all terms, conditions and provisions of the Lease shall remain in full force and effect. In the event of any conflict between the provisions of this addendum and the Lease, the provisions of this addendum shall govern.

Smart lock and Microwave Installation Service Agreement

VIDA UNIT UPGRADES
Parties: VIDA COMMUNITIES INC. ("VIDA") and the Tenant under the Lease to which this agreement is attached as a Schedule (collectively, the "Parties”).
Background:
VIDA is the agent responsible for the residential property of this tenant.
The Tenant is currently leasing the Property pursuant to a signed lease agreement.
The Tenant has applied and been approved by VIDA to upgrade the existing entranceway locks at the Property to smart locks and receive a countertop microwave in accordance with the terms and conditions contained herein.
Agreement Terms and Conditions:
1a. Installation of Smart Locks: VIDA agrees to install a smart lock on the primary entranceway door of the unit. The specific brand and model of the smart lock shall be determined by VIDA in its sole discretion.
1b. Installation of Microwave: VIDA agrees to install a countertop microwave in the unit of the tenant. The specific brand and model of the microwave shall be determined by VIDA in its sole discretion.
2. Service Fee: The Tenant agrees to pay an additional monthly fee of $18.99 for the smart lock (the "Smart Lock Service Fee") and $15.99 for the microwave (the “Microwave Service Fee”) which fee shall be added to the Tenant’s monthly rent payable under the Lease.
3. Payment: The Smart Lock Service Fee and the Microwave Service Fee shall be due and payable at the same time as the Tenant's monthly rent. Failure to pay the Smart Lock Service Fee in full each month shall be considered a default under the Lease.

The tenant agrees to clean the microwave regularly after use to keep it neat and prevent grime from forming in the microwave.
5. Security: The Tenant acknowledges that while the smart lock is designed to enhance the security of the Property, VIDA makes no guarantees or warranties about the effectiveness of the smart lock in preventing unauthorized access to the Property.
